Hello, I work as a Registered Nurse with the Guelph Family Health Team. Chronic Disease management and illness prevention is a large priority with our FHT. I am here to learn more about how I can support my patients through workplace initiatives in managing the chronic care population. Hi everyone! My name is Marie. I graduated from Ryerson University in 2010 with a BScN and minor in psychology. I have been working at Sick Kids since I graduated, the last 4 years in their Medical/Surgical ICU. In addition, I have nursing experience in a small town hospital North-West of Toronto, being a clinical instructor and international experience in South Africa and Dominican Republic. I am currently enrolled full time in the Primary Health Care Nurse Practitioner MN program at York University and started in the fall of 2015. I plan on working in a small community and look forward to proving holistic care to all ages, but I have an interest in women’s health, mental health, global health, and bettering chronic disease management in the community while supporting caregivers. I have seen the complex management some diseases require from family members and want to provide my patients with better education around their diagnosis, management and prevention of further complications. Since my nursing experience is mainly with acute care, I am hoping this course will provide me with further knowledge in chronic disease management that will better prepare me in providing outstanding care for my patients in my community and in a global context. Hello all. My background includes visiting nursing, public health, acute hemodialysis, and palliative care. Since Oct 2013 I have been employed part time at St James Cathedral as a parish nurse. This role is all about health in it's fullest sense encompassing physical spiritual, emotional, social, psychological. Rather than hands on care I try to be a resource and a catalyst . I do pastoral care, education, advocacy, nurturing networks of support, helping people cope, helping people navigate systems. The website can tell you anything you need to know about parish nursing. It looks to me like this webinar will strengthen my professional practice.

I work as a Diabetes/Chronic Disease Primary Care lead 8 hrs per week at the North East LHIN. I have joined here to build my professional network and knowledge.
